چکیده

Twenty-five patients with trigeminal neuralgia have been treated by fractional posterior fossa Vth nerve section. The procedure is shown to be effective in relieving pain, preserving corneal sensation and the motor root. Nineteen patients showed little loss of light touch sensation, and in 15 of these there was dissociation of sensory loss to pinprick and light touch. The results support the concept that there is some anatomical separation of fibres subserving light touch and pain in the posterior root of the nerve.

The treatment of trigeminal neuralgia by posterior fossa microsurgery.

Between 1972 and 1981 57 patients underwent posterior fossa exploration in Oxford by a single surgeon for the treatment of trigeminal neuralgia. Fifty-four of these had either partial or total section of the trigeminal sensory root, 2 had microvascular decompression operations and one had both a partial sensory root section and microvascular decompression. There was no mortality and no signific...

Operative findings on microsurgical exploration of the cerebello-pontine angle in trigeminal neuralgia.

The anatomical findings in 52 patients undergoing posterior fossa exploration for idiopathic trigeminal neuralgia are described. Anatomical abnormalities in the cerebello-pontine angle were found in 46. The commonest was an arterial loop indenting or distorting the nerve at the root entry zone. In view of this the operative procedure has been changed from root section to microvascular decompres...

Trigeminal Neurinomas: A Review of a Personal Series

Trigeminal neurinomas are relatively rare tumors and represent 0.2% of all intracranial tumors. (1–12) Trigeminal neurinomas usually arise from the Schwann cells of the sensory root and can originate in any section of the fifth cranial nerve; correspondingly, a variety of symptoms and signs may develop. Improved surgical outcome and long-term growth control have been uniformly reported after th...

Cerebellopontine epidermoid presenting with trigeminal neuralgia for 10 years: a case report

Trigeminal neuralgia, also called tic douloureux, is a common and potentially disabling pain syndrome, which affects the trigeminal or fifth cranial nerve. The precise pathophysiology of Trigeminal neuralgia remains obscure. The disorder causes extreme, sporadic, sudden burning or shock-like face pain that lasts from few seconds to minutes and can be physically and mentally incapacitating. More...

Ultrasound-Guided Pulsed Radiofrequency Application via the Pterygopalatine Fossa: A Practical Approach to Treat Refractory Trigeminal Neuralgia.

Although pharmacological therapy is the primary treatment modality for trigeminal neuralgia associated pain, ineffective analgesia and dose limiting side effects often prompt patients to seek alternative pharmacological solutions such as interventional nerve blockade. Blockade of the Gasserian ganglion or its branches is an effective analgesic procedure for trigeminal neuralgia, traditionally p...
